Treatments & Pre-Service Questionnaires

A pre-service questionnaire is required with each new client and new service. Please advise your service provider if you are pregnant, have injuries or recent surgeries. If you are using Retin-A, Accutane or other medications, please inform your therapist so appropriate products and treatments are selected for your spa service. Due to active ingredients used in some treatments, clients are advised when redness and peeling may occur after treatment. This questionnaire can be downloaded from our web site or sent by mail for clients wishing to complete the form prior to arrival.
