Preparing for the Spa
The Medical Spa at Nova understands that each individual has their own wellness concerns, health conditions that need consideration and goals they wish to obtain from their selected spa services. For this reason we ask all our guest to complete a medical consent form before we begin any treatment. If you are concerned or have questions as to which treatments and therapies are best for your total wellness and personal goals we are able to schedule you a free consultation where you can meet with a spa staff member.

Spa Forms
When preparing for your spa visit, if you would like to fill out your medical consent forms prior to arriving at The Medical Spa at Nova, the following forms are available to download as PDF documents here;

Medical Health History Form - required for all services and treatments.

Chemical Peel Forms Packet - required for all Chemical Peels. If you are having a Vitalize Chemical Peel, please complete the Vitalize Packet.

Ear Candling Consent Form - required for ear candling treatments.

Community Acupuncture Health History Form - required for all Community Acupuncture participants along with either the male or female health history form.

InvisaLift Consent Form - required for InvisaLift treatments.

IPL Consent Form Packet - required for IPL treatments.

Laser Hair Removal Packet - required for all Laser Hair Removal treatments.

Laser Skin Tightening & Laser Vein Therapy - required for Laser Skin Tightening and Laser Vein treatments.

Microdermabrasion Packet - required for microdermabrasion treatments.

Parafango Consent Form - required for Parafango treatments.

Personal Training and Fitness Class Consent Form - required for all new fitness clients.
